synopsis/solicitation for commercial items/services prepared in
accordance with format in sub parts 12/6 and 13.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation to be issued, proposals are being
requested from this announcement, a written solicitation will not be
issued. The Request for Proposal number is 575-439-98. This
solicitation incorporates provisions and clauses in effect through FAC
97-02. Standard Industrialization Code (SIC) of 8999 is applied. This
solicitation is issued subject to the availability of government funds
at time of award. This solicitation is unrestricted for the following
service: Contractor shall provide maintenance services for the
following Toshiba Ultrasound equipment: SSH-140A Ultrasound System with
peripherals, SSA-270A Ultrasound System with peripherals. Maintenance
is to include two (2) preventive maintenance visits per year, all parts
and labor during business hours (8:30-5 Monday thru Friday). The
following provisions and clauses are applicable and are hereby
incorporated into the solicitation by reference and will be provided by
the contracting officer in full text upon request: FAR 52.212-1,
Instruction to Offerors Commercial Item applies. Addendum clauses FAR
52.215-18 Facsimile Proposals also applies. Offerors are required to
complete and include a copy of the following provisions with their
proposals: FAR 52.212-3, Offeror Representations and Certifications
Commercial Items, FAR 52.212-5, Affirmative Action for Special Disabled
and Vietnam Era Veterans; FAR 52.222-36, Affirmative Action for
Handicapped Workers; FAR 52.222-37, Employment Reports on Special
Disabled Veterans and Veterans of the Vietnam Era. Award will be made
to that offeror (1) whose proposal is technically acceptable and (2)
whose technical/cost relationship is most advantageous to the
Government. While cost is secondary to technical, it will be a factor
in the award decision. Also, award may not necessarily be made for
technical capabilities that would appear to exceed those needed for the
successful performance of the work. Proposals from responsible offerors
