is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. Solicitation is being issued
as Request for Quote (RFQ) 80051002. The solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-01. This requirement is being set aside for
small business for which the Standard Industrial Classification Code is
4953 and Small Buisiness Standard of $6.0 million. Line Item 0001: Job
Service/Non-Personal: Contractor shall provide all parts, labor,
equipment, material, supervision and transportation necessary to
transport and dispose of approximately 50,000 lbs. of non-hazardous,
non-controlled medical substances which have been damaged in storage or
have exceeded applicable shelf life criteria. Contractor shall burn and
dispose of material in accordance with all State, Federal and Local
laws or regulations pertaining to the proper pick-up, transportation
and disposal of non-hazardous, non-controlled medical substances
requiring class X disposal (incineration). Contractor shall be
available for pick-up of all non-hazardous, non-controlled Medical
substances currently on hand between the hours of 8:00 am to 1:30 pm
Monday through Friday (California time). If the pick-up date falls on
a holiday, pick-up of material will be accomplished the following work
day. Material shall be packaged in tri-wall containers (48"L x 40"W x
42"D) and/or palletized on pallets, unit weight of tri-walls/pallets
range from 400 to 1000 pounds each, average loads will be 10 to 40
pallets per shipment. Contractor shall provide Certificate of
Destruction for contract acceptance/completion. Material located at
Defense Distribution Center West, Tracy Facility, Whse 10, Section 2,
Tracy, California. Contract Delivery/Completion Date: 30 days after
receipt of order. Only FOB Destination pricing will be accepted.
