Lancaster Road, Dallas, TX 75216 has a requirement for one (1) Community Based Outpatient Clinic in each of the following Texas counties: Johnson and Ellis. The contractor shall provide for the delivery and management of continuous primary and preventive medical health care for all eligible Veteran Affairs enrolled patients. The contractor shall provide health providers, medical facilities, medical equipment and supplies, short-term pharmaceutical prescriptions and administrative support. Compensation will be made through a monthly capitation payment for each patient in accordance with the terms and conditions of the proposed contract. The issue date for the solicitation will be on 02/15/98: the proposal due date will be on 03/18/98.
