Ondriezek, 605-347-7027 The VA Black Hills Health Care System and Sioux Falls VA Medical Center intends to award contract(s) for Community Based Outpatient Clinics (CBOC) to provide Primary Care Services on an outpatient basis for approximately 1200 veterans residing in the Pierre, South Dakota and surrounding area. The CBOC is to be located within the city limits of Pierre, South Dakota. The CBOC will be responsible for managing the veterans' primary medical care. Referrals for sub-specialty care and hospitalization will be made to the VA's located in Sioux Falls, South Dakota or Black Hills Health Care System (Fort Meade or Hot Springs, South Dakota). It is expected that the contract(s) will be awarded on or before March 17, 1998 with services to begin on or about July 1, 1998 for a one year period with renewal options. Offers must be received by COB March 4, 1998. Offers are being solicited on a capitated basis, fee for service or the leasing of space.
