The Contractor shall provide all necessary personnel, routine drugs, medical supplies, equipment, aircraft, ground transportation to and from the airport, and malpractice insurance to provide emergency air medical transport to Navajo Area Indian Health Service (NAIHS), Contract Health Service (CHS) eligible patients referred to the Contractor by authorized personnel at Fort Defiance PHS Indian Hospital, Fort Defiance, AZ. Services shall be available 24 hours/day, 7 days a week in the States of Arizona, New Mexico, Colorado and Utah. Listed below are the annual estimated quantities: Level I, multi-engine/fixed wings, 183 Flights; Level I-Rotor Craft, 23 Flights; and Level II-Air taxi, single engine/ fixed wings, 1 Flight. The Contractor shall respond within 60 minutes of the estimated time of arrival (ETA) to the facility. Anticipated award will be for a twelve month period with two (2) one-year options.
