AMENDMENT: Previous announcement under Request For Proposal No. 97-16/JC is cancelled and replaced with RFP No. 98-08/VZ -- General/Medical Surgical Services/ACLSU. This acquisition is released as a 100% Small Business Set-Aside, as a non-personal service contract. Services are required in the ambulatory clinic and inpatient setting of the Acoma-Canoncito-Laguna Indian Hospital, San Fidel, New Mexico. A single contract will be awarded for a base year and four, one-year options for renewal. All proposals received will be evaluated and considered by the Government. Proposal will be evaluated based on numerically weighted technical evaluation factors. Evaluation of Technical Proposals and Cost or Price Proposals are of approximately equal value. Prospective contractor must possess the following and must be submitted with proposal package: Availability to meet the required deliverables, license in one of the 50 States, show evidence that he/she is currently active in surgical work, two letters of reference to include one from Chief of Staff where individual is a member of the medical staff and one from another practicing surgeon, contractor or staff must have proof of surgical residency, curriculum vitae must show that the work record includes a minimum of two years experience in rural community or services provide to a minority population, with consistent dates.
