noted solicitation (submitted January 28, 1998), is amended as follows:
(1) In the third sentence of the description, change solicitation
541-85-98 to read "541-117-98". (2) Definitions/Acronyms -- a. VISN 10
= Veterans Integrated Service Network 10, which includes the following
VA Medical facilities, VA Chillicothe, Cincinnati,
Brecksville/Cleveland and Dayton. DRG = Diagnostic Related Group. (3)
The following information is intended to clarify and/or provide
additional information necessary to complete quotations and said
additions in no way deletes any of the previously stated requirements:
a. Contractor shall be required to write a program (create a
micro-based data base, using a standard relational database such as
MS-Access, or SQL which shall permit the tracking and reporting of
information for each mental health provider in VISN 10). The central
computer system shall be located at the VA Medical Center, 10000
Brecksville Road, Brecksville, OH 44141, systems design shall be such
that all other VISN 10, Network Mental Health Service Lines/Providers
shall have ability to download required forms and reports into the
Brecksville central system. Contractor shall be required to coordinate
development of reports and form formats with the COTR which will then
produce standard microsoft access reports/forms. Contractor shall be
allowed to develop program at the contractor's facility, however,
contractor shall be required to perform a site visit in order to
acquire an understanding of the VA computer system. Contractor shall be
required to install said program on VISN 10 central computer system.
Contractor shall provide training to VISN 10, Network Mental Health
Service Line Provider's. Contractor must have capability of moving a
large quantity of data out of the current central computer and into a
microsoft relational database. Period of performance shall be 120
calendar days from date of award, to include development, installation
and training of VA personnel. Notwithstanding any other provisions,
the Government intends to award without discussion to the responsible
offeror whose offer conforming to the solicitation will be most
advantageous to the Government, both technical and price considered.
Therefore it is in the best interest of the offeror to provide its'
most advantageous offer initially. However, the Government reserves the
right to hold discussions should it be deemed necessary. Contractor
shall be required to submit a technical and price proposal, which shall
be concise and provide sufficient information to demonstrate the
offeror's capacity to satisfactorily perform the tasks outlined in the
RFQ, and shall address the following factors in descending order of
importance. a. Technical -- Program Development -- to include Knowledge
and ability to program Microsoft Access or Microsoft SQL; Knowledge and
ability to take a large amount of data from the centralized computer
and converting it to Microsoft Access or Microsoft SQL; Understanding
of VA Requirements; Past Performance. b. Management Approach -- to
include years experience programming in Microsoft Access and Microsoft
SQL; experience of personnel performing actual program development;
ability to develop comprehensive and complete programming. c. Price --
reasonableness. d. Compliance to VA schedule -- ability to develop,
install and train VA personnel with the scheduled 120 calendar days.
The due date for receipt of quotations is changed from February 13,
1998 at 4:30 p.m., to now read February 24, 1998 at 4:30 p.m. All other
