Officer (Site Code DADA15) (i) This is a combined synopsis/solicitation
for commercial items prepared in accordance with the format in FAR
12.6, the test program in FAR Subpart 13.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. (ii) This solicitation
document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular 90-45. (iii) Walter Reed Army
Medical Center intends to negotiate with Medtronic, Diagnostic
Division, on a sole source basis for a base period and four one year
option periods. (iv) QUANTITIES AND PRICING: Pricing shall be provided
for the following quantities. Provide quantity discounts when
applicable. (v) ITEMS: Contractor shall provide the following
instrumentation at NO additional charge to the Government: Two (2) each
HMS Instruments, CAT# 305-02, One (1) each ACT II Instruments, CAT#
400-15, and One (1) each ACTime Electronic, CAT# 313-35. Contractor
shall provide preventive maintenance, emergency maintenance and repair
on the instrumentation during the term of this contract at no
additional cost to the Government. The contractor shall not be
responsible for damage to the instrumentation caused due to willful
misconduct on the part of Government employees. The contractor shall be
responsible for troubleshooting all assay or quality control problems
encountered during the use of these assays, provide a qualified
technical service office which can resolve such problems via telephone.
The contractor shall include all instrumentation upgrades at no
additional cost to the Government that occur during the effective
period of this contract. The contractor shall retain ownership of the
above instrumentation throughout the term of this contract and shall
remove instrumentation from the Government facility upon expiration of
the contract. The Government shall not be responsible for any cost
associated with returning instrumentation to the contractor. All
expenses for returning the instrumentation after the contract period
has expired shall be borne by the contractor. In the event the
contractor does not remove the instrumentation from the Government
facility within 30 working after date of contract expiration, the
Government shall remove and ship collect to the contractors place of
business. Contractor shall deliver, on the first Tuesday of each month,

(hemoSTATUS), CAT# 304-79, (6) Hepcon HMS Coagulation Control Pack,
CLOTrac HR Control Pack, CAT# 550-13, (7) Hepcon HMS Coagulation
Control Red/Yellow, CAT# 306-01, (8) Hepcon HMS Coagulation Control
Tan/Silver, CAT# 306-02. Option periods are as follows: First Option
Period: 98 Oct 01 -- 99 Sept 30, Second Option Period: 99 Oct 01 -- 00
Sept 30, Third Option Period: 00 Oct 01 -- 01 Sept 30, Fourth Option
Period: 01 Oct 01 -- 02 Sept 30. Item descriptions, delivery date, and
quantities shall remain constant throughout all exercised option
periods. Contractor shall provide pricing for all option periods. (vi)
Date of delivery is no later than 30 days after notice of award. Place
of delivery is Walter Reed Army Medical Center, Building 2, Room 4203,
6825 16th St., NW, Washington, DC 20307. FOB Destination. (vii)
Solicitation provision at FAR 52.212-1, Instructions to
Offerors-Commercial Items (OCT 1995) is hereby incorporated by
reference. (viii) All responsible bidders may submit a written offer
for consideration by the Government. A firm fixed priced contract
resulting from this solicitation, will be awarded by the Government to
the responsible offeror whose offer conforming to this solicitation is
most advantageous, representing the best value to the Government, price
and other factors considered. The following factors shall be used to
evaluate offers: Evaluation will be based on (1) Technical (a)
Technical capability of the item offered to meet the Government
requirement, and (b) Delivery Terms Ability to meet required delivery
schedule and (2) Price. Factors (1) and (2) are of equal importance.
(iv) Offeror's are reminded to include a completed copy of the
provision at 52.212-3, Offeror Representations and
Certifications-Commercial Items (JAN 1997), with its offer. (x) Clause
52.212-4, Contract Terms and Conditions-Commercial Items (MAY 1997),
is hereby incorporated by reference. (xi) Clause 52.212-5, Contract
Terms and Conditions Required to Implement Statues or Executive
Orders-Commercial Items (AUG 1996), is hereby incorporated by
reference. (xii) The Defense Priorities and Allocations System (DPAS)
