71 -- SPECIALIZED CUSTOM MEDICAL FURNITURE AND RELATED CUSTOM CASEWORK SOL RFQ 796-98-04 DUE 030998 POC Mick Dubuis 813-792-7593 This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number Request For Quotation (RFQ)796-98-04 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-03. The Standard Industrial Classification Code is 3821and the small business size standard is 500 employees. This announcement and a list of pre-existing furniture and add-on furniture that the government still requires, will constitute the only solicitation: quotations are being requested and a full written solicitation will not be issued. To obtain the previous mentioned list, potential offerors are encouraged to contact the point of contact listed in this notice. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-03. Under the authority in FAR 6.302-1 (ii) the James A. Haley VA Hospital intends to negotiate a sole source procurement with Milcare, a Herman Miller company for add-on hospital grade (anti-microbial) furniture and related casework. This furniture is to be in strict accordance with strict infectious control standards. All furniture must be consistent with hospital appearance and 100% compatible with the existing Milcare furniture. Furniture is to be designed for, delivered too, and installed at Vandenberg Air Force Base located at 30MDG/SGSL 338 S. Dakota Ave., Building 13850, Vandenberg AFB, CA 93437. If submitting a quote on a brand -name equal; adequate technical information shall be included. This information shall include: the quality assurance documents for infectious control, technical specifications, product availability, product picture, product features, etc. Quotations shall be submitted on an "all or none" basis. Partial quotations or quotations without sufficient descriptive data shall be rejected. All quotations shall state delivery time, F.O.B., and shipping data if F.O.B. is origin. All potential offerors shall be available to perform modifications immediately. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
