This is a combination synopsis/solicitation prepared in accordance with
Federal Acquisition Regulation Subpart 12.6 for commercial items as
supplemented with additional information included in the notice. This
announcement constitutes the only solicitation; quotations are being
requested and a written solicitation will not be issued. This
solicitation is issued as a Request for Quotation (RFQ) using
solicitation number F0561198T0629. This acquisition is for two each
Field II Analyzers w/power table, model 750, manufactured by Humprey
Instruments, 2992 Alvarado St., San Leandro, CA
94577-0700,800-423-4393. This is an approved sole source acquisition
action as authorized by AFFAR SUPP 5313.106 under Simplified
Acquisition Procedures. No Request for Quote (RFQ) package will be
issued. However, all interested sources may identify their product by
submitting a complete quote, to include pricing data, an accurate
description of the equipment offered and make and model numbers of the
equipment in accordance with the instructions listed below. Only
responses containing this data will be considered. If no favorable
responses are received by close of business 15 days from the date this
notice appears in the CBD, a purchase order will be negotiated with
the above named source. This solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-03. The Standard Industrial Classification Code is 3827
and the business size standard is 500 employees. Technical
specifications for the two each Humphrey Field II Analyzers w/ Power
Table, model 750, Instrumentation is completely self contained with
on-board computer system, memory and data storage. instrument has UL,
CSA and IEC safety approvals, instrument is provided with wheelchair
accessible power table with built-in 8 1/2 x 11 printer, instrument is
upgradeable with new software revisions, true projection perimeter
with projection device capable of projecting light stimulus onto a 30cm
radius projection bowl with Goldman standard bowl illumination of 31.5
apostles, (not LED or Fiberoptic), compact ergonomic size to fit short
or heavy set patients, projection stimulus capable of projecting
Goldmann standard I, II, III, IV, V, (1,2,3,4,5) size targets, color
testing capability in white, red, or blue, stimulus intensity range to
10,000 apostles with a stimulus duration of 200 mesc, touch screen and
keyboard user interface with CRT display, Video Eye Monitor system for
eye alignment and observation plus automatic blind spot tracking, Gaze
tracking system with histogram display providing real time evaluation
of the eye's gaze position independent of head alignment, head tracking
system to automatically align head position during the test to maintain
eye centering in the trial lens, Vertex monitoring system to alert
operator if patients head position has moved back from the forehead
rest to prevent inducing trial lens ring artifact, patient data input
capability to record the following information: name ID, number, trial
lens Rx, visual acuity, pupil size, IOP, C/D ratio and patient
comments, automatic pupil size determination capability, Statpac 2
analysis printouts to compare patient's test results to extensive data
base of normal patients, Glaucoma change probability printout to
compare test results to extensive data base of stable glaucomatous
patients to indicate change over time in patients with glaucoma,
Fastpac (r) software to reduce visual field testing time of threshold
tests by 40%, Swedish interactive threasholding alagrarithim, automatic
pupil size determination capability, all plugs/cords must be hospital

requirement : All information technology items must be year 2000
compliant. Year 2000 compliant means information technology that
accurately processes dates/time data (including, but not limited to,
calculating, comparing, and sequencing) from, into and between the
twentieth and twenty-first centuries, and the years 1999 and 2000 and
leap year calculation. Futhermore, year 2000 compliant information
technology, shall accurately process date/time data if the other
information technology properly exchanges date/time data with it.
