synopsis solicitation for commercial services prepared in accordance
with the format in FAR Subpart 12.6 as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation Request for Quotations (RFQ) being requested and a
written solicitation will not be issued. Solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular Number 97-03. This procurement is set aside for
small business and the SIC Code is 8742 and the small business size
standard is $5.0 million. Services are required for a JCAHO pre-survey
consultation utilizing the 1998 Joint Commission on Accreditation of
Hospital Organizations (JCAHO) standards. Contractor shall provide all
labor materials and supervision to perform a JCAHO pre-survey during
the months of April -- May 1998 for the VA Medical Center,
Fayetteville, N.C. The pre-survey shall address Medical Staff and
Nursing and an assessment shall be made of the facility's compliance
with JCAHO standards. The medical center is a 154 bed general medicine,
surgery, and psychiatric facility that provides primary and secondary
care. It also houses a 39-bed nursing home care unit. For 1997, the
medical center recorded in excess of 140,000 outpatient visits and
3,700 discharges. The proposal should be based on the JCAHO priorities
for a facility of this size and complexity. The HAP component of the
survey should be a key component in the proposal preparation. The
pre-survey shall be performed through the use of interviews, document
review, tour of patient care areas or other techniques. At the
conclusion of the pre-survey, an exit conference shall be conducted to
present a summary of findings and discuss areas of noncompliance and
provide recommendations for compliance. A comprehensive written report
outlining the findings and recommendations shall be provided to the VA
within two weeks following completion of the survey. The actual dates
of the pre-survey shall be coordinated with the Contracting Officer's
Technical Representative. Services shall be performed Monday through
Friday during the hours of 8:00 a.m. to 4:30 p.m. excluding Federal
holidays. It is required that the Contractor and contractor personnel
have actual JCAHO survey experience. The following provisions apply to
this acquisition: 852.219-70 Veteran-Owned Small Business; 852.270-4
Commercial Advertising; 52.212-1 Instructions to Offerors-Commercial
Items; 52.212-3 Offer Representations and Certifications-Commercial
Items; 52.212-4 Contract, Terms and Conditions- Commercial Items;
52.212-5 Contract Terms and Conditions to Implement Statutes or
Executive Orders- Commercial items, including clauses incorporated by
reference under Section (b)(6) through (9); 52.212-2 Evaluation --
Commercial Items; Offerors responding to this announcement shall submit
their cost to perform services (all inclusive rate), a prospectus
identifying how and what services will be provided and the time frame
in which services will be provided. Offerors should submit information
or examples of similar contracts performed for government agencies
within the last three years and names and phone numbers of points of
contact for past performance. Award will be made to the offeror whose
offer conforming to the solicitation will be most advantageous to the
Government, price and other factors considered: Experience,
qualifications and cost factors will be used to evaluate offers.
Experience and qualifications, when combined, are significantly more
important than cost or price. The information required in provisions
52.212-1, a completed copy of provision 52.212-3 and the certification
required in 852.219-70 must be submitted with offer. Quotations must
be received by March 18, 1998 at 4:30 p.m. local time. Offerors
