Q -- PRIMARY CARE SERVICES SOL RFP: 671-60-98 DUE 041098 POC Carol Sienkiewicz (210)617-5102, Sandy Farrow (210)617-5151 Provide Primary Care to veterans assigned to the contractor by the South Texas Veterans Health Care System (STVHCS). Contract will be based on an all inclusive capitated rate. Initial primary care visits will consist of physician's office visit including medical history and physical exam, clinical assessment diagnosis and treatment. Specialty consultation and more extensive diagnostic workups (e.g., Cardiology, Pulmonary, Dermatology, and Computerized Axial Tomography) shall be referred to the STVHCS. The contractor will be responsible for supplying new or emergent prescriptions. The contractor must be able to supply Pharmacy, Laboratory, and Radiology services. The site for this contract will be Uvalde and the contractor is required to be physically located within this city. Services are to be performed at the contractor's site. Contract is for one year from date of award with two (2) additional one-year options. Solicitation document will be available on or about March 10, 1998.
