Procurement of this survey on an other than full and open competition basis is anticipated from Joint Commission on the Accreditation of Health care Organizations. This requirement is a combined synopsis/request for quotation for commercial item prepared in accordance with FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only request for quotation: a written quotation will not be issued. Responses shall contain sufficient documentation to establish a bonafide capability to fulfill the requirements. The request for quotation document number is N62645-98-Q-1002. The request for quotation and purchase order incorporates provisions and clauses that are in effect through Federal Acquisition Circular 97-03. The standard industrial classification (SIC) code is 8071. The small business size standard is $5M. JCAHO will satisfy the requirement by providing the following Contract Line Items (CLINs): CLIN 0001, Surveys for Naval Hospital and 2 branch clinics at Sigonella, Italy; CLIN 0002, Waiver testing for Lab; CLIN 0003 Travel/per diem for surveyors. Offeror shall complete survey by 31 Mar 98. The provision 52.212-1, Instructions to Offerors-Commercial Items, applies and offers shall be evaluated for fair and reasonable price and technically acceptability. Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with their offer.
