The intent of this contract is for the Contractor to provide for the delivery and management of primary and preventive medical care and the continuity of that care for all VA enrolled patients. The Contractor will provide health care clinical providers, medical facilities, medical equipment and supplies and all administrative functions to fulfill the level of care and range of services. The Contractor will provide case management services by primary care providers who function as case managers for all health care services provided to enrolled patients.
