Synopsis/Solicitation. Request for Quotation #RFQ-618-111-98-LK
entitled "Video On-Demand System" is hereby issued for commercial items
and is prepared in accordance with the format in FAR Subpart 12.6 as
supplemented by additional information included in this notice and
incorporates provisions and clauses in effect through FAC 97-04. The
SIC Code is 3663 and the small business size standard is considered 750
employees. This CBD announcement constitutes the only solicitation
issued. A separate written solicitation document will not be issued.
Contractors' proposals are hereby being requested and response to
requirements listed below shall be submitted on contractor's letterhead
and must address all requirements listed below and should contain the
name, title and signature of person authorized to submit proposals/bids
on behalf of the contractor. Descriptive literature for each item
offered must also be included in proposal/bid package. The VA Medical
Center (VAMC), Minneapolis, Minnesota, has a requirement for a Signal
Video Distribution System that will distribute patient and staff
educational video programming on an on demand basis throughout the
medical center. System required must be capable of: automated storage,
retrieval, and playback of video tape programming; simultaneous
multi-user/multi-program access; scheduled broadcasting and automated
recording; multiple interface and control options; interactive video
control; and provide remote access from any network location. (The VAMC
has a video capable network in place.) The system components and
services required shall consist of the following line items or equal:
(1) 1-each Video Library Manager capable of holding a minimum of 8
VCR's and 200 video tapes (a TiltRac Model F-124-II or EQUAL) (2)
1-each System Computer and Software Windows NT with 4 line telephone
input card and Network (TCP/IP) Interface Card (3) 1-each Hewlett
Packard Office Laser Jet Printer or EQUAL (4) 1-each Four Channel
Character Generator System (5) 4-eachFour channel Frequency Agile
Modulators for 12 channels (6) 9-each JVC SR3360U VHS VCRs or EQUAL (7)
1-each Panasonic CT 1386VY Television/Monitor or EQUAL (8) 1-each APC
1000VA UPS (9) Head end distribution equipment, installation and MATV
system balancing (10) On site system installation and setup (please
specify number of days required along with cost (11) Staff/Patient
testing option on Windows NT (12) System Operation Training for 2
system administrators and 2 backup system administrators (please
provide number of days required for training, site of training and
cost) (13) System Maintenance training for two VA Biomedical Technology
Technicians (provide number of days of training, location, cost (14)
The VAMC will serve as a beta test site for ongoing development of
Patient Survey and Testing Packages (15) Contractor shall perform a
radio frequency site survey to ensure that the current VA system is
capable of handling the multi-channel video signals. This report should
be given in writing and should include requirements and any
recommendations for upgrade, if necessary (16) 2-each
Service/Maintenance manuals for each of the electronic components
ordered (17) Warranty Period for parts and labor (please specify term
of warranty) Delivery shall be FOB Destination (unless vendor states
otherwise) to the VA Medical Center, Warehouse (90D), One Veterans
Drive, Minneapolis, MN, 55417. This will be a firm fixed price
contract. Evaluations may be conducted to determine that products
offered will meet Government's needs. Evaluation factors of technical
capability of the items offered and past performance history will be
considered along with price to determine "best value" to the
Government. Evaluation may be made with or without discussions with the
offerors. The following incorporated FAR clauses and provisions are
those in effect through FAC 97-04 and are applicable to this
acquisition. 52.212-1 Instruction to Offerors Commercial Items;
52.212-2 Evaluation Commercial Items; 52.212-3 Offeror Representations
and Certifications Commercial Items (NOTE: Offerors must include a
completed copy of "Offeror Representations and Certifications" along
