The Substance Abuse and Mental Health Services Administration (SAMHSA) through the General Acquisitions Branch, PSC, plans to award a four month sole source simplified acquisition to the National Planned Lifetime Assistance Network (PLAN) Alliance in Saratoga Springs, NY. Work under this contract is to plan and convene a three-day conference of the member programs in relation to the following objectives:(1) provide a status report on all existing 22 PLAN programs (2) present examples of PLAN programs that are active and providing the "safety net" for family members including benefits to consumers as a result of the PLAN programs (3) provide an opportunity for new families to engage PLAN family members in a dialogue about PLAN (4) develop a strategy for future care planning as public policy and (5) formulate a national/regional strategy for expanding and funding PLAN.
