ext. 1403 This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. this
announcement constitutes the only solicitation: quotes are being
requested and a written solicitation will not be issued. (ii.) This
Solicitation number MDA410-98-Q-0017 is issued as a request for quote.
(iii.) This solicitation document and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular #90-45
final rule effective 1/97. (1) The standard industrial classification
(SIC) code for this acquisition is 8732. (2) This small business size
standard is 5 Mil. . List of contract line Item numbers: CLINs 0001:
Commercial Off the Shelf (COTS) customer satisfaction instruments that
will assess parents, students, and teacher's/staff's perceptions of
the quality of services DoDEA provides: The Department of Defense
Education Activity intends to enter into a Firm Fixed price contract to
provide 144,000 surveys (70,000 to parents, 70, 000 to students, and
40,000 to Faculty) of 100 randomly selected schools to be administered
during the month of May, 1989. The contractor will score and analyze
results and provide DoDEA with reports. The contractor will ship the
instruments directly to each school's point of contact, along with the
policies and procedures for conducting the study. The schools will be
given three weeks to administer and collect the completed surveys. The
contractor will receive the instruments for scoring from the schools in
bulk form, i.e., parent, student and teachers/staff surveys will be
sent together. The contractor will input all information into a
database using an optical scanner. The contractor will provide to DoDEA
Headquarters, districts and schools the appropriate reports as
delineated in "Work to be Performed." Five meetings between the
contractor and the government will be incorporated within the scope of
the contract. The contractor will score and analyze results and
provide DoDEA with reports and data tables for each school and
district. The data tables will include the percentages of responses for
each question, broken out by gender, grade level, etc. The contractor
will also provide an executive summary, report and data tables for all
of DoDEA to be provided to DoDEA Headquarters. The contractor will
brief DoDEA Headquarters on the results of this study. The project will
be completed by September 18, 1998. The sequence of events is as
follows: A. The contractor will provide DoDEA with off-the-shelf
customer satisfaction surveys that assess the perceptions of DoDEA's
parents, students and teachers/staff no later than 3 weeks after
contract award. The provided surveys will have a range of 75-150
questions. The government will have the opportunity to add up to 10
questions per instrument. B. The government will randomly select 100
schools using a stratified sampling process in which these surveys will
be administered. The government will provide the contractor with the
sample. C. The contractor will ship the surveys, in bulk, to a point of
contact at each school. The contractor will include in the shipment
policies for administering the surveys. Surveys will be received in the
schools no later than 5 weeks after contract award. D. The point of
contact in each school will be responsible for the distribution and
collection of the surveys to and from the parents, students and
teachers/staff at each school. The schools will be given three weeks to
complete the surveys. E. The contractor will receive the surveys, in
bulk, from each school. The surveys will be received by the contractor
no later than 10 weeks after contract award. F. The contractor will
enter each survey into a database no later than 10 weeks after contract
award. G. The contractor will provide no later than 25 weeks after
contract award.: (a) an overall, DoDEA report, including an executive
summary, (b) an overall report for each district, including an
executive summary, (c) a summary report for each school, (d) one set of
data tables which include the percentages of responses for each
question broken out by gender, grade level, etc. for each school and
district, (e) five separate sets of data tables which include the
percentages of responses for each question broken out by gender, grade
level, etc. will be produced for the Arlington DoDEA office, (f) a
final briefing of these results at DoDEA Headquarters in Arlington, VA
to include briefing slides. A. An electronic version of the database
of survey responses. B. Five meetings between the contractor and the
government will be incorporated within the scope of the contract. (iv)
FAR 52,212-1, Evaluation Commercial Items, is incorporated by
reference and applies to this acquisition. Addendum. 52.212, Par. (a).
the following factors shall be used to evaluate offerors; past
performance and price. Contractors must have an off the shelf product.
Offeror's shall include a complete copy of the provision 52.212-3,
Offeror Representations and Certifications -- Commercial Items, with
it's proposal. FAR 52.212-4 "Contract Terms and Conditions --
Commercial Items," is incorporated by reference and applies to this
acquisition and resulting contract. the following terms and conditions
are added as an addendum to this clause: Type of Contract: The
resulting contract shall be firm fixed price. FAR 52.212-5, Contract
Terms and Conditions Required to Implement Status or Executive Orders
-- Commercial Items (Oct. 1995) is incorporated in this contract by
reference, however for paragraphs (b) and (c) only following provisions
