a combined synopsis/solicitation for commercial items prepared in
accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This notice shall
constitute the only solicitation and a written solicitation will not be
issued. This Basic Ordering Agreement shall provide initial evaluation
services for veterans who establish basic eligibility under Chapter
31, Title 38, U.S.C. as authorized by a case manager from the
Vocational Rehabilitation and Counseling (VR&C) Division of the VA
Regional Offices in Cleveland; Cincinnati; and Columbus. Services
include but not limited to taking a client's history, evaluating the
veteran's rehabilitaiton assets and liabilities, making recommendations
regarding appropriate rehabilitations services and expected outcomes
and summarizing all information, evaluations and recommendations in a
comprehensive report. The client history shall include, but will not be
limited to personal and family history; education/training history;
military service; employment history; financial circumstances;
service/non-service connected disability conditions; any potential
barriers to successful vocational rehabilitation. Contractor shall
provide names and qualifications of individuals who will perform
services with their proposals. Contractor shall schedule the initial
intake counseling session within 7 days of authorization by the VA Case
Manager. Invoices may be submitted weekly, biweekly or monthly in
arrears. Contractor's place of business shall be based in Ohio and will
provide services as needed in Hamilton County at locations designated
by VA. VA contract service review representatives are authorized to
visit the premises of the contractor during business hours for the
purpose of evaluations; auditing and inspecting records. All
comprehensive written initial evaluation reports are to be provided to
the VA within 7 days of the completion of counseling services.
Proposals shall be evaluated on the following criteria and evidence to
support the evaluation criteria must be submitted with your proposals.
Evaluation Criteria: (1) Previous experience and present experitise in
vocational rehabilitation counseling and initial evaluation of disable
veterans to include: number of disable veterans seen previously for
counseling and initial evaluation; type of services provided;
approximate date (by year) of cases by category of services; Agency or
counseling setting in which services were provided; Average number of
sessions by type of services provided; An indication of the length of
time services were provided, by case and any other relevant
information. If the above experience was done as a federal government
contractor it may be worth 100 points. (2) A committment in principle
to providing services to disabled veterans as evidenced by it's bylaws,
mission statement, or established statement of goals and procedures.
(10 points). (3) An established program, recording the client's
history; evaluating the client's rehabilitation assets and liabilities;
making recommendations regarding appropriate rehab services and
expected outcomes. (50 points) (4) The use of individualized plans for
rendering services to clients. Documentation may include any forms
used in recording the client's history; evaluating the veterans rehab
assets and liabilities making recommendations regarding appropriate
rehab services and expected outcomes and summarizes all information,
evaluations and recommendations in a comprehensive report. (10 points).

Contractor's personnel shall meet the following minimum education
requirements: doctoral degree in voc rehab rehab counseling;
counseling; psychology; counseling psychology; social work or another
closely related field from an accredited institute and/or a master's
degree in vocational rehab counseling or a master's in psychology,
counseling, counseling psychology, social work or another closely
related field from an accredited institute. Also professional
credentials of (CRC) or (LPC) or (LPCC) licensed from the State of
Ohio. (100 points) The following provisions apply to this solicitation
and may be requested in writing to the Contract Specialist listed
above. Provisions are 52.212-1, Instructions to Offerors-Commercial;
52.212-2, Evaluation-Commercial Items; 52.212-3, Offeror
Representations and Certifications; 52.212-4, Contract Terms and
Conditions-Commercial Items and 52.212-5, Contract Terms and Conditions
required ti implement Statutes or Executive Orders. This procurement is
100% small business set-aside. Two (2) copies of proposals shall be
submitted to the above mentioned Contract Specialist. No faxed copies
will be accepted. The VAMC shall award Basic Ordering Agreements in
rank order with the first award going to the offeror that receives the
highest number of points during the evaluation process. The VAMC also
reserves the right to award none, one or additionals on the basis of
need in a descending order of rank. Please note that the proposals with
the lowest price may not necessarily be awarded. Period of performance
shall be from April 1, 1998 to March 31, 1999. Proposal shall include
