Woods, 301-443-0043 The Center for Mental Health Services is soliciting proposals from qualified small business organizations to award a one-year contract with three one-year options to assist States/Territories in the effective operation and monitoring of the Projects for Assistance in Transition from Homelessness (PATH) program. The purpose of this contract is to support the collection, synthesis, and dissemination of program-related information (through meetings, distribution of written information, on-site technical assistance sessions and teleconferences) designed to meet the needs of the State agencies which are responsible for the administration of this program. The contractor will prepare briefs describing exemplary practices and develop issue papers to improve the delivery of services to persons who are homeless and have serious mental illnesses. The contractor shall also provide technical assistance to State mental health authorities and local providers of mental health and other services for persons who are homeless and have serious mental illnesses. The successful offeror must be knowledgeable and have a track record of accomplishments in providing technical assistance at the State and local delivery system level related to homelessness and serious mental illness. It is essential that the personnel and consultants involved in this contract exhibit competencies appropriate to the unique needs and complexities of homeless persons who have serious mental illnesses. It is essential that the activities undertaken in performance of this contract be carried out with input and participation from representatives of the State and Territorial mental health authorities, including national organizations which represent State/Territorial mental health interests. Further, the contractor must provide opportunities for involvement from consumers and/or family members of consumers of PATH services, i.e. those persons who have mental illnesses and are homeless or at risk of becoming homeless. Request for Proposal (RFP) No. 280-98-8003 is scheduled for issuance on or about 15 days after publication of this notice and will close 45 calendar days thereafter. This solicitation may be obtained two ways: electronically or by mail. Electronically: The file may be downloaded from the SAMHSA Web Site at http://www.samhsa.gov. Files will not be available on the Web Site until on or after the estimated issuance date. Once downloaded, you should determine if you have a continuing interest in the RFP. If so, you must check the SAMHSA Web Site regularly for any amendments to the RFP which will also be posted there and can be downloaded. We cannot assume responsibility for assuring that you have downloaded the amendments. Therefore, you may wish to write to the Contract Specialist listed below to request that copies of the amendments to the RFP (if any) be sent to you by mail. By Mail: Requests for copies of the RFP to be mailed must be made in writing or by facsimile (301/443-0019), and will be honored if received within twenty days after issuance ofthe solicitation. Requests received after this period will be accepted on a first-come, first-served basis until the supply is exhausted. There is no assurance that copies requested after the twentieth day will reach the requester before the due date for receipt of responses. Send written requests to Mary E.
