COURSE The Government intends to negotiate a firm fixed price contract
for the instruction of a course for General Industry Course as
required by the Naval Occupational Safety and Health, and Environmental
Training Center, Norfolk, VA. The resulting contract will be for the
base period of 01 October 1998 through 30 September 1999, with 4 option
years. All responsible sources may submit offers, resumes and past
performance data which shall be considered by this agency. This is a
100% small business set-aside. The Standard Industrial Code is 8299. In
order to streamline the acquisition process, resumes and past
performance data must be submitted to the Fleet and Industrial Supply
Center by 2:00 PM 08 April 1998. This information is due prior to the
submission of your written offer. One instructor is required to teach
this course. Instructor must have: (A) a four (4) year baccalaureate
degree and four years of experience in OSH/crane operations OR a
minimum of ten (10) years of experience in OSH/crane operations; (B) a
minimum of three (3) years of instructional experience; AND (C) of the
three years of instructional experience, at least one (1) year of
experience must be in teaching Naval personnel OR at least three (3)
years of active duty in the Navy with an honorable discharge. Offerors
shall submit resumes with the following information: Name and address
of instructor candidate; Degree(s) held, name of institution awarding
the degree(s), and date degree(s) awarded; Description of work
experience that contributes to successfully teaching this course;
Amount of teaching experience and a description of where this
experience was gained; Amount of Navy experience, where this experience
was gained and how this experience will contribute to successfully
teaching this course; Awards earned by the instructor candidate, and/or
any professional certifications held, i.e., Certified Industrial
Hygienist, Certified Safety Professional. Offerors shall describe their
past performance on similar contracts held within the last three (3)
years which are of similar scope, magnitude and complexity to that
required by this solicitation, or affirmatively state that they possess
no relevant directly related or similar past performance. Offerors who
have held similar contracts shall provide a detailed explanation
demonstrating the relevance of the contracts to the requirements of
this RFP and shall provide the following information: Contract
number(s); Name and number of COR, or point of contact at the activity
for which the contract was performed; Dollar value of the contract;
Detailed description of the work performed; Number of subcontractors
used, if any, and a description of the extent of work performed by
them. The Government shall assess the risks associated with offeror's
past performance and consider the number, type and severity of any
quality, delivery or cost problems in performing the contract, the
corrective action taken and the effectiveness of the corrective action.
If resumes are determined to be unacceptable by the technical
evaluation committee, offeror will not be given the opportunity to
correct the resumes. Only those offerors whose resumes are determined
to be acceptable will continue to Part II-Evaluation for Award. Part II
consists of the technical proposal which will be an oral presentation;
the price proposal; and past performance. Requests for the
solicitation must state that neither the requestor nor the principal
corporate officials and owners are currently suspended, debarred, or
otherwise ineligible to receive contracts from any Federal Agency.
Failure to so represent may be grounds for refusing the request.
