Veterans Affairs Medical Center, Omaha, Nebraska has a requirement for
Orthotic appliances and related services to those veterans that require
them and who are eiligible VA beneficiaries. It is VA Omaha's intention
to solicit proposals and award multiple contracts on a competitive
basis to preferred providers within the VA Medical Center, Omaha,
Nebraska jurisdiction. Orthotic appliances include custom made/custom
fit braces, corset/belts, elastic hose, arch supports, shoe inlays,
molded shoes, and orthotic shoes. The contractor shall fabricate all
orthotic appliances in strict conformance to the prescription that has
been proviced by the VA physician. All contractors shall have
full-time American Board Certified (ABC) orthotists on staff at each
location. The contractor's facilities shall be approved by the Facility
Accreditation Program of the American Board for Certification in
Orthotics and Prosthetics, Inc., or have successfully passed inspection
by VA. RFP 636-24-98 has been assigned to this proposal. Contract
period will be May 15, 1998, through May 14, 1999, and two option
years. This solicitation will be prepared in accordance with the format
in the Federal Acquisiton Regulation Part 12 for Commercial Items and
a written solicitation will be issued on or about April 10, 1998. All
requests for this proposal must be in writing and may be faxed to
