Q -- SKILLED NURSING HOME HEALTH CARE, SKILLED THERAPY, BOWEL AND BLADDER CARE, HOMEMAKER/HOME HEALTH AIDE, AND INFUSION SERVICES SOL 539-06-97 DUE 052398 POC Betty Jo Dessauer, 513/475-6340 or Judith Blasingame, 513/475-6336 Contractor shall furnish skilled nursing home health care, skilled therapy, bowel and bladder care, homemaker/home health aide, and infusion services to patients referred by the Department of Veterans Affairs Medical Center Cincinnati. The primary service area for this Medical Center are Dearborn, Franklin, Ohio, Ripley, and Switzerland counties in Indiana; Boone, Bracken, Campbell, Gallatin, Grant, Kenton, and Pendleton counties in Kentucky; and Brown, Butler, Clermont, and Hamilton counties in Ohio. The VA reserves the right to make one or multiple awards. Offeror shall be able to provide all services to the entire Primary Service Area. A single contractor may be awarded a contract for one or more services. Offeror and all subcontractors shall have a current JCAHO Letter of Accreditation and Certified as a Home Health Agency. Shall also be Medicare certified and a managed care provider. The contract period shall be a base period from the date of award through 9/30/98 with one prepriced option to renew contingent upon the availability of funds. Issue date O/A 4/1/98.
