Q -- PRIMARY CARE CLINIC SOL RFP 635-62-97 DUE 050698 POC Beverly Kimball, Contracting Officer This VA Medical Center has a requirement to utilize a pre-existing clinic in the Ponca City area complete with physicians and personnel for primary care and routine phamaceutical services for the purpose of providing primary care services via a community-based clinic in the location of Ponca City, Oklahoma. At the time of response to this solicitation, clinic must already be JCAHO accredited and have either on-site lab, x-ray, or pharmaceutical capability OR at least be able to demonstrate that a contractual relationship exists to provide off-site lab, x-ray, or pharmaceutical services to the existing clinic. Clinic must be located within a 30 mile radius from the center of Ponca City, Oklahoma and must be located within this medical center's primary service area. Physicians and technicians operating under the resulting contract must meet all evaluation criteria and other professional certifications required to operate in such a clinic. The qualifications of such personnel shall also be subject to review by the Chief of Staff of this medical center and approved by the Oklahoma City VA Medical Center Director. Clinic profiles will be established as clinic staff deems appropriate to accommodate workload demand. Solicitation issue date shall be on or about April 6, 1998.
