synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6 as suplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; Request for Quotations (RFQ) are being requested
and a written solicitation will not be issued. Solicitation document
and incorporated provisions are those in effect through Federal
Acquisition Circular 97-3. The SIC Code is 3845 and the small business
size is 500. Contractor shall provide on site maintenance services at
the VA Medical Center, 10701 East Boulevard, Cleveland, Ohio, for
fourteen (14) Model 7200 Series Ventilators. Services shall include
on-call maintenance and a preventive maintenance inspection every 2,500
hours and every 10,000 hours. Each PMI performed shall include all
inspection, parts, electrical safety test and a full peformance
verifcation test. Contractor shall provide all necessary labor,
equipment, tools, supplies, materials, parts, technical manuals,
supervision, transportation and lodging to perform contract services.
All defective components will become the property of Contractor and be
removed at Contractor's expense. All parts provided shall be new,
current manufacture parts, compatible with existing equipment.
Contractor shall be available to perform services during the work hours
of 8:00 a.m. to 4:30 p.m., local time, Monday through Friday excluding
weekends and government holidays. Contractor shall arrive on site to
perform services within four (4) hours after notification and proceed
progressively to complete services without undue delay. All work shall
be performed by competent technicians, experienced and qualified to
work on specific equipment. Work performed shall be in accordance with
commercial practices and the equipment manufacturer's specifications.
Contractor shall make all adjustments and repairs resulting from
manufacturing defects and ordinary wear and tear (excluding externally
caused damage or negligence by the VA). Heshall obtain all necessary
licenses and/or permits required to perform this work. He shall take
all precautions necessary to protect persons and property from injury
or damage during the performance of this contract. He shall be
responsible for any injury to himself, his employees or others as well
as any damage to personal or public property that occurs during the
performance of this contract that is caused by his or his employees'
fault for negligence, and shall maintain personal liability and
property damages insurance prscribed by the Laws of the State of Ohio.
The Contractor shall provide certificates or other forms of
documentation that indicates the service technician responsible for the
equipment maintenance has received training to service the equipment
contained in this contract. Contractor shall provide and install
circuit revisions and rectify identified FDS (Food and Drug
Administration) device hazards for all equipment listed. Contractor
shall provide all diagnostic test equipment, test fixtures and software
diagnostic programs to input and service equipment contained in this
solicitation. If a part, system component, assembly or subsystem
covered under this contract is replaced and the replacement part is
incompatible with contracted equipment software, the contractor shall
provide and, if necessary, license the VA for compatible software at no
charge. This software is considered a replacement item coincidental to
a contract covered repair of the unit and is deemed a replacement for
the incompatible software. The software shall remain with the
equipment and is considered government property. Payments will be made
upon the receipt of a invoice in arrears. All invoices must be
supported by service reports on record in the Biomedical Engineering
Office, Rm. B06, Wade Park Unit, VA Medical Center, 10701 East
Boulevard, Cleveland, Ohio 44106. Period of performance shall be from
April 16 through September 30, 1998 with three (3) pre-priced option
years. The following provisions apply to this acquisition: 52.212-1

and 52.217-9 Option to Extend the Term of the Contract with a thirty
(30) day written notice given and contract duration not exceeding a
period of 3 years, 6 months. Copies of these provisions may be
requested in writing to the Contract Specialist listed above. Bidders
responding to this announcement shall submit two copies of quotations
with their monthly maintenance price for the basic year period and
three one year option periods to the attention of the above mentioned
