838-6088 This is a combined synopsis/solicitation for commercial items
prepared in accordance with the format in FAR Subpart 12.6, as
supplemented with additional information included in this notice. This
annoncement constitutes the only solicitation; quotations are being
requested and a written solicitation will not be issued. Solicitation
No. 541-152-98 is being issued as a Request for Quotation. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-03. Standard
Industrial Classification Code is 3699 and the Small Business Size
Standard is 750 employees. Contractor shall provide a Full Service
Maintenance Contract for the Siemens Mevatron KDS2 Linear Accelerator,
including all Vacuum Product Upgrade located at the Wade Park Unit, VA
Medical Center, 10701 East Boulevard, Cleveland, Ohio 44106. Services
shall include total coverage for preventative and emergency maintenance
as specified herein, including parts, labor transportation, lodging,
and all other allied costs associated with the performance under this
contract. All defective components will become the property of
Contractor and be removed at Contractor's expense. Contractor shall
make all adjustments and repairs resulting from manufacturing defects
and ordinary wear and tear (excluding externally caused damage or
negligence by VA). He shall obtain all necessary licenses and/or
permits required to perform this work. He shall take all precautions
necessary to protect persons and property from injury or damage during
the performance of this contract. He shall be responsible for any
injury to himself, his employees or others as well as for any damage to
personal or public property that occurs during the performance of this
contract that is caused by his or his employees' fault for negligence,
and shall maintain personal liability and property damages insurance
prescribed by the Laws of the State of Ohio. All work shall be
performed by competent technicians, experienced and qualified to work
on specific equipment. All work performed shall be in accordance with
commercial practices and the equipment manufacturer's specifications.
Repair of damage or malfunctioning caused by massive power failure,
accidents, negligence, misuse or abuse, weather conditions, or any
other not associated with the reasonable and normal use of the
equipment, or originating outside of the equipment is not required of
Contractor. During the period this agreement is in effect, no personnel
other than employees of the Contractor wil perform service or
maintenance on the equipment unless specifically authorized by
Contractor and approved by the Contracting Officer. Contractor shall be
available to perform services during administrative work hours, (Monday
through Friday 8:00 a.m. to 4:30 p.m. excluding Weekends and Government
Holidays). Emergency Service Onsite Visits: When contacted by
authorized VA personel for repair of faulty equipment, the Contractor
will arrive at the hospital with four (4) hours to service the
equipment and perform all work necessary to enable the equipment to
function within the manufacturer's specifications. Contractor will
perform four (4) scheduled preventive maintenance inspections per year
(i.e.,calibration, cleaning, lubrication, adjustments, replacement of
components requiring periodic replacement) to the equipment
manufacturer's specifications. Payments will be made upon receipt of an
invoice in arrears. Period of performance shall be from May 1, 1998
through September 30, 1998 with three (3) pre-priced option years as
follows: Option Year One: October 1, 1998 through September 30, 1999;
Option Year Two: October 1, 1999 through September 30, 2000; Option
Year Three: October 1, 2000 through September 30, 2001. The following
provisions apply to this acquisition: 52.212-1, Instructions to
Offerors-Commercial items; 52.212-2, Evaluation-Commercial Items;
52.212-3, Oferor Representations and Certifications; 52.212-4, Contract
Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and
Conditions Required to Implement Statutes or Execturive
Orders-Commercial Items and 52.217-8 Option to Extend Services and
52.217-9 Option to Extend the Term of the Contract with a thirty (30)
day written notice given and contract duration not exceeding a period
of 3 years, 5 months. Quotations must be submitted in writing to the
attention of the Contracting Officer listed above. No faxed copies will
