combined synopsis/solicitation for commercial items, prepared in
accordance with the format in FAR subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation;quotations are being requested and a
written solicitation will not be issued. The solicitation number,
N00259-98-Q-0120, is issued as a request for quotations(RFQ). The
solicitation document and incorporated provisions and clauses are those
in effect through FAC 97-04. The order will be firm-fixed price,
indefinite delivery type purchase order. GAMMA KNIFE services is a
package service which includes the following: CLIN 00001: The Technical
and the Physician component. The technical component includes: use of
the contractor's Leksell G stereotatic frame and the localizer
frames(elektra instruments, inc., frame application, a CT or an MR
targeting study(adding cerebral angiography and additional charge),
treatment planning using the Gamma unit computer and Gamma Plan
software,treatment using Gamma Knife(Elektra Instruments,Inc) one night
hospitalization,ambulance service to and from contractor's imaging
facility and the services of their medical physicist, their
neurosurgical nurse while at the Gamma Unit suite. 6 packages. CLIN
00002: Angiography Services,2 packages. CLIN 00003: Anesthesiology
Services, 2 packages. CLIN 00004: Radiologist Fee, 6 packages. The
STATEMENT OF WORK : 1. Maintain Joint Commission on Accreditation of
Healthcare Organizations(JCAHO),American Association of Blood Banks
(AABB),College of American Pathologists(CAP),and American College of
Radiology(ACR)accreditation. 2. Provide facilities, (WITHIN 15-20 MILES
RADIUS)medical staff and support personnel(employees), ancillary
support,diagnostic and therapeutic services, equipment, and supplies
necessary for the proper care and management of patients under this
agreement, which as a minimum must include but not limited to: a.) The
use of the Leksell G stereotatic frame and imaging localizer
frames,frames application, and either a CT or a MR Targeting study. b.)
The services of the gamma unit neuroradiologist for establishing the
protocol for MRI studies and/or performing cerebral angiography if
required by targeting. c) The use of appropriate patient monitoring
devices during treatment planning and treatment. d) Treatment planning
using the gamma unit computers and gamma Plan software. e) The
services of the gamma unit medical physicist as well as the services of
the gamma unit neurosurgical nurse during gamma knife treatment
planning and treatment. f.) One night hospitalization if determined
necessary by the Medical Treatment Facility (MTF) attending physician.
g) Clinical and anatomical pathology support for MTF patients being
treated at the contractor's facility shall be provided exclusively by
the contractor's affiliated providers of such services. h)Professional
and technical radiology support for MTF patients being treated at
contractor's facility or it's affiliated providers. j) Anesthesiology
services for MTF patients being treated at contractor's facilities
shall be provided exclusively by the incumbent or its affiliated
providers. 3. Provide administrative support to participating uniformed
service health care practitioners and if necessary,including,a)
Maintenance of patient records,including transcription and copying
services necessary to satisfy both Department of the Navy and the
contract's record keeping,quality assurance,risk
management,accreditation,and medical board requirements. b.)Reasonable
accomodations within the contractor's facility for such periods of
time as the participating uniformed service health care practitioner
maybe providing care in the contractor's facility. These accomodations
shall be provided at no additional costs to the government. 4. Be
solely and exclusively liable for any and all acts or ommissions
amounting to professional negligence on the part of it employee
providing services under the non-personal service contract. While the
U.S. government will be similarlysolely and completely liable for any
and all acts or omissions amounting to professional negligence on the
part of its employees,in the event a Federal Tort Claims ACT claim,
other claim,or lawsuit is brought or filed against the U.S. Government
may implead, cross claim against or otherwise seek contribution from
the contractor. The contractor will provide full professional liability
insurance for any and all causes of action arising from its performance
or that of its employees in the amount that is usual and customary in
the civilian practice in the community. Verification of this liability
insurance shall be provided as part of the non-personal service
contract. 5. Be responsible for verifying professional liability
insurance coverage for all civilian members of the contractor's medical
staff who may assist the participating uniformed service health care
practioner . This includes,but is not limited to,
radiologists,pathologists and anesthesiologist. 6. Adhere to all MTF
claim submission requirements for institutional charges for which it
bills. The contractor shall not bill for the professional services of
the uniformed practitioners. OTHER CONSIDERATIONS: In the event of
unforseen cicumstances,NMCSD will have the right to reschedule
contracted services at no additional charge to the government. This
acquisition incorporates the following FAR clauses: 52.212- 1
INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS(oct 95);52.212-2 EVALUATION-
COMMERCIAL ITEMS (oct 95)is incorporated by reference with the
following addenda: in paragraph (a) the government will evaluate
proposals based on PRICE,TECHNICAL AND PROFESSIONAL capability and cost
to the government. All other factors, when combined, is approximately
equal to cost/price; 52.212-3 OFFEROR REPRESENTATIONS AND
CERTIFICATIONS-COMMERCIAL ITEMS (oct 95) must be completed and
submitted along with the proposal (please contact Loida Toledo at
