This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. RFQ-R5-13-98-17, this number
must be referenced on all communications and proposals. The request
for quotations document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97-04. This is a
total small business set-aside. The Standard Industrial Classification
is 7372 with a size standard of $18.0 million dollars. One each Bar
Code System for Mobilization of Porterville Organized Crews --
Overview: The current system being used to mobilize a maximum of 600
emergency firefighters is accomplished by having the firefighters
report for sign-up, request theiridentification card and then manually
typing their name, social security number, gender, personal weight,
cargo weight, and pay rate line by line for a total of 18 firefighters
onto a Passenger and Cargo Manifest which is a four-part carbon-copy
form. After the manifest is completed, the next step is to manually
enter the header information on each firefighter onto the Emergency
Firefighter Time Report. A minimum of 30 minutes must be allowed to
accomplish this task for each crew and is done so with a minimum of 4
persons. Product Requirement: This Bar Code System shall include the
Software, Photo-ID System for employee badge identification, Hand held
Scanner or Slot Reader, and training for the user. This system should
be able to complete the following steps. Step 1: Each employee shall
have a 8 x 5 1/2" file card. The card contains information pertinent to
that employee, which consist of a Photo, Name, Address, City, Phone
number, social Security Number, Date of birth, Body weight,
Citizenship, Visa Expiration. date, I-9 U.S. Immigration Naturalization
verification, Fire Training Accomplishment, Emergency Notification,
Certificate of Medical Examination, Certified chainsaw Operator, and
Fire eligibility. Step 2: Each firefighter shall have a 3 1/2 x 2"
identification badge of which he carries on his person throughout the
fire season. The information on the badge is a header, agency,
expiration of medical or physical, name, address, city, weight, date of
birth, approving officer signature, and a photograph of the employee.
Step 3: The next step of this Bar Coding System would be the printing
of the firefighters (badge) personal information onto a Emergency
Firefighter Time Report OF-288, for each firefighter. Only Block 2
through 14 and column A would be required to be completed using this
system. Step 4: The Bar Code System needed shall have the capability of
productivity, printing the personal information, (name, social security
number, etc.) for each firefighter onto a SF-245, until the manifest is
completed, with 18 to 20 firefighters. Copies of existing cards and
forms may be requested from Nancy Hargis at (209) 784-1500, extension
1240. Responses to this CBD notice should include proposed cost, time
of delivery, and technical information/descriptive literature. The
following clauses are included as part of this request for quotation:
Clause 52.212-1, Instructions to Offerors -- Commercial Clause
52.212-2, Evaluation-Commercial Items. The Government will award a
contract resulting from this solicitation to the responsible offeror
whose offer conforming to the solicitation will be most advantageous to
the Government, price and other factors considered. The following
factors shall be used to evaluate offers. 1. Technical capability to
meet Governments's requirements, 2. Past Performance. 3. Price. These
criteria are listed in descending order of importance. Technical and
past performance, when combined, are more important than price. A
written notice of award or acceptance of an offer, mailed or otherwise
furnished to the successful offeror within the time for acceptance
specified in the offer, shall result in a binding contract without
further action by either party. Before the offer's specified expiration
time, the Government may accept an offer (or part of an offer), whether
or not there are negotiations after its receipt, unless a written
notice of withdrawal is received before award. Offerors shall include
a completed copy of provision 52-212-3 Offeror Representations and
Certifications -- Commercial Items. Clause 52-212-4 Contract Terms and
Conditions -- Commercial Items. 52-212-5 Contract Terms and Conditions
