combined Synopsis/Solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation. Quotes are being requested and a
written solicitation will not be issued. This Solicitation Number,
RFQ646-98-3-934-0143, is issued as a Request for Quote (RFQ). The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-04. The standard SIC
code is 7372 and the small business size standard is $18.0 M. The
Department of Veterans Affairs VISN #4 has a requirement to procure a
commercial software package of Physician Fee Data and Capitation
Manager. SPECIFICATIONS: (1)Database shall contain as a minimum, but is
not limited to: Medical/Surgical Procedures by Current Procedural
Terminology (CPT) code. Physician's Fee for each procedure at 6 levels
for each specified geographic areas. Levels are: Resonable & Customary
(highest reasonable charge observed); Managed Indemnity (amount
typically reimbursed by commercial payers); High Managed Care (high end
of managed care plans' negotiated fee ranges); Typical (mid-point of
managed care plans' negotiated fee ranges); Low Managed Care (low end
of managed care plans' negotiated fee ranges) and Medicare (current
year fee amount for locality corresponding to geographic area published
by authorized Medicare carriers with HCFA's Relative Value Units used
to develop Medicare RBRVS fee. Geographic Area as a minimum, shall
encompass catchment area of VISN #4. This includes the states of
Delaware and Pennsylvania; Atlantic City and Southern Region in New
Jersey; Binghamton Area and Chautauqua County in New York; Baltimore,
Eastern, Northern, and Northwestern Regions in Maryland; Ashtabula,
Belmont, Columbiana, Guernesy, Harrison, Jefferson, Monroe, Morgan,
Noble, Washington, and Athens Counties in Ohio; Barbour, Braxton,
Brooke, Calhoun, Doddridge, Gilmer, Hancock, Harrison, Jackson, Lewis,
Marion, Marshall, Monongalia, Pleasants, Preston, Randolph, Ritchie,
Roane, Taylor, Tucker, Tyler, Upshur, Webster, Wetzel, Wirt, and Wood
Counties in West Virginia. Geographic area is to be referenced by zip
codes on database. (2) Capitation Manager Utility shall identify
specific per member/year rates for negotiations with managed care
companies and with outpatient facility physicians. (3) Database and
Utility shall be provided on CD-ROM or 3-1/2" High Density Diskettes
(delimited) in standard ASCII file format. (4) Support Services shall
consist of Periodic Data Updates (minimum of every 6 months or more
frequently); Unlimited Telephone Consulation for each VISN location
with an 800 number telephone access; Up to 100 allowable Requests as a
minimum, for Research/Verification of Non-Standard Medical Procedural
Codes for new procedures, market variances and/or market trends; Up to
100 allowable Requests as a minimum, for Additional Medical Procedural
Codes for specialties and/or geographic areas outside licensed
specialties and/or market areas. (5) Scheduled Training for software
and utility, including operating and service manuals required for
updates. Contract is for 1 year with two 1-year options to renew by the
government. There are 6 items for each period: Base Year
(5/1/98-4/30/99), Option Year 1 (5/1/99-4/30/00), Option Year 2
(5/1/00-4/30/01). All items are as specified above. ITEM 1. Eleven
CD-ROM/Disketts with Physician Fee Database, with password to access.
ITEM 2. Capitation Manager. ITEM 3. Support Services. ITEM 4. Updates
ITEM 5. Training. Delivery is FOB Destination to VAMC's of VISN #4 at
Altoona, Butler, Coatesville, Erie, Lebanon, Philadelphia, Pittsburgh,
Wilkes-Barre in Pennsylvania; Clarksburg in West Virginia and
Wilmington in Delaware. Acceptance is upon confirmation by all
destinations corroborating receipt and CD-ROM/diskettes and password.
Provision at 52.212-1, Instructions to Offerors -- Commercial, applies
to this acquisition with the following addenda. Facsimile offers and
modifications are permitted. Provide information supporting adequacy of
proposed software to fulfill specifications and firm's capability to
provide support services. Submit a price for each item, total price of
all items for the base year, option year 1 and option year 2 with a
summary total of all years combined. FAR Clause 52.212-2 is
incorporated by reference and applies to this acquisition. Three
factors shall be used to evaluate offers: Adequacy of proposed software
to satisfy or excel specifications for software and updates; Capability
of firm to provide support services; and Price. Adequacy and
Capability, when combined, are approximately equal to price. Those
responding are to include a completed copy of the provision at
52.212-3, Offeror Representations and Certifications -- Commercial
Items and VAAR Provision 842.219-70, Veteran-Owned Small Business with
their quote. Quotes are due on April 23, 1998 by 4:30 PM (local Pgh.
