Provide Case Management Services to Eligible Beneficiary Veterans in the Omaha, Nebraska, metropolitan area, for the period of July 1, 1998, through June 30, 1999, and for two additional option years. Solicitations are to be issued April 15, 1998. All proposals are to be received by close of business May 15, 1998. Requests for solicitations are to be in writing. Specific services shall include (1) an orientation of the veteran to the VA Vocational Rehabilitation Program, (2) Problem solving counseling, (3) Supervision of the veteran's program or rehabilitation to include personal meetings at specified time periods one time per term, (4) Recommendations and coordination of changes in the IWRP (Individualized Written Rehabilitation Plan) in conjunction with the veteran and the VA Counseling Psychologist, (5) Recommendations and coordination of support services, including training site officials, treatment providers, and community resources, (6) Recommendation and coordination of changes in training site or rate of pursuit, and (7) Provision of professional and clerical support services associated with case folder documentation of actions taken.
