This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.06, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation shall not be issued. Solicitation #F62321-98-T2099-Item 0001 -- Request for Quotations. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-3. SIC code -- 3949. This acquisition is for a playground system gym manufactured by Game Time, part number 11513-KSE-1 or equal. Gym shall have aluminum uprights. The unit consists of 4 ea. rectangular metal decks, 1 ea. arch climber. Equipment shall include: 2 ea. age appropriate(5-12 years of age) sign/panel with frame -"Welcome" sign: maintenance plan manual identifying frequency and procedures for inspecting each piece of equipment; detailed installation instructions and drawings; and appropriate warning signs. Equipment shall comply with CPSC 1991 Handbook for Public Playground Safety and ASTM F1487-93 Standard Consumer Safety Performance Specification for Playground Equipment for Public Use. Delivery shall be at the earliest time possible but not later than 1 June 1998. For shipments that can be made via US Mail, delivery shall be FOB Destination -- APO, AP 96368. US Mail is the preferred shipping method. For shipments that cannot be accomplished via US Mail, delivery shall be FOB Destination -- Lathrop, CA. In addition to the guidance contained in FAR 52-212-4, contractors shall submit proof of delivery with their invoices. Proof of delivery may be in the form of a bill of lading or other shipping document signed by the receiving office. Invoices received without proof of delivery shall not be accepted.
