65 -- ULTRASOUND SOL RFQ 580-32-98 DUE 051898 POC Contracting Officer, Cathy Edwards, (713) 794-7423 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Request for Quotation number for this solicitation is 580-32-98. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-04. The standard industrial classification for this procurement is 3845 and the small business size standard is 500 employees. Provide the following FOB Destination to the VA Medical Center in Houston, Texas within thirty days of receipt of order. Line item 1 -- Clearview Ultra Instrument with Automatic Pullback Device, SCIMed Life System #I5006 or equal. To be considered equal, the offered ultrasound must possess the following salient characteristics: Mechanical transducer, digital sagittal reconstruction of image, and an automatic pull-back device. Quantity: 1 each. Line item 2 -- Operating manual for item 1. Quantity: 2 each. Line item 3 -- Service manual for item 1. Quantity 1 each. Price should include hardware and software upgrades that improve ease of use or image quality improvements during first twelve months from ship date of the instrument. Price should include set up, inservice and user training (as well as any applicable travel costs for training). Provisions of clause 52.212-1, Instructions to Offerors -- Commercial Items and addenda that apply follows: 52.214-21 Descriptive Literature, 852.210-70 Requirement for Operating and Maintenance Manuals, 852.210-71 Guarantee clause (contractor guarantees the equipment against defective material, workmanship and performance for a period of one (1) year from date of equipment acceptance by the Government), and 852.210-77 Brand Name or Equal. Evaluation of quotes will be based on the technical capability of the item offered to meet the government's needs and on price -- in that order.
