Holloway, Contracting Officer (970) 242-0731 X2037 This is a combined synopsis/solicitation for commercial items/services prepared in accordance with format in subparts 12.6 and 13.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation to be issued, proposals are being requested from this announcement, a written solicitation will not be issued. The Request for Proposal number is 575-450-98. This solicitation incorporates provisions and clauses in effect through FAC 97-02. Standard Industrialization Code (SIC) of 8099 is applied. This solicitation is issued subject to the availability of government funds at time of award. This solicitation is unrestricted for the following service: Contractor to provide anesthesiology services for this VA Medical Center for the period of approximately May 18, 1998 through July 31, 1998. Services will include 24 hours a day, 7 day a week anesthesiology coverage. This service will include the actual administration of all forms of anesthesia as well as overseeing the CRNA's. Contractor will be responsible for assuring all medical record documentation relating to anesthesia, including pre and post anesthesia assessments, history and physical examinations, progress notes, etc. are done in accordance with JCAHO standards and medical center medical staff bylaws and policies. All responsible sources may submit a proposal which shall be considered by the agency. The following provisions and clauses are applicable and are hereby incorporated into the solicitation by reference and will be provided by the contracting officer in full text upon request: FAR 52.212-1, Instruction to Offerors Commercial Item applies. Addendum clauses FAR 52.215-18 Facsimile Proposals also applies. Offerors are required to complete and include a copy of the following provisions with their proposals: FAR 52.212-3, Offeror Representations and Certifications Commercial Items, FAR 52.212-5 Affirmative Action for Secial Disabled and Vietnam Era Veterans; FAR 52.222-26, Affirmative Action for Handicapped Workers: FAR 52.222-37, Employment Reports on Special Disabled Veterans and Veterans of the Vietnam Era. Award will be made based on past performance and the qualifications of the candidate. Although cost is secondary it will be a factor in the award decision.
