Q -- COMMUNITY BASED OUTPATIENT PRIMARY CARE SERVICES SOL RFP 648-40-98 DUE 060598 POC Mary Cray, 503-273-5375 The Portland VA Medical Center (PVAMC) located at 3710 SW U.S. Veterans Hospital Road, Portland, Oregon, requires primary care services on an outpatient basis for approximately 630 current veteran users residing in nine counties of Central Oregon: Crook, Klamath, Deschutes, Lake, Grant, Wasco, Harney, Wheeler, Jefferson. The community based provider will be responsible for managing the eligible veterans' primary medical care. Referrals for sub-specialty care and hospitalization will be made at the Portland VA Medical Center or as otherwise designated. To be considered for award, the offeror must demonstrate (1) existed as a business entity providing similar services for a minimum of two years as of the date of proposal submission; (2) provide certification that all providers for the required services (including services to be provided by subcontract arrangements) are licensed by the appropriate governing body; (3) have a minimum of one facility located within a 30-minute drive of Bend, Oregon. Evaluation criteria is described in the Solicitation. It is expected that contract(s) will be awarded the third week of June 1998 with services to begin within 30-45 days from contract award. The term of the contract will be for one year with a one-year option to extend. A commercial item request for proposal will be issued on or about May 7, 1998.
