The areas in the counties that requires servicing are : a)Nassau, b)Western/Mid Suffolk County, c) Eastern Suffolk County-North Folk, d) Eastern Suffolk County-South Fork. Services are required for each of the areas noted above. Accordingly, a separate proposal must be submitted for each area in which the offeror proposes to provide services, and the offering individuals or organization must have one or more locations within each defined area. Required, will be a base price for individual sessions and price for group therapy. Only responsible sources may submit a quotation. Only written requests will be accepted.
