Q -- OPTOMETRY SERVICES SOL RFP672-169-98 DUE 052598 POC Natividad Ruiz 787-766-5559 This is a solicitation for commercial items prepared in accordance with format in subpart 12.6 and 13.6 for optometry services for the VA Medical Center, San Juan, Puerto Rico. Contractor must be within the geographical area of San Juan and have access points in Ponce and Mayaguez, Puerto Rico. Services include eye examinations, prescription eyeglasses with case, eyeglass fitting and refraction services to the VA beneficiaries. Optomotrists must have a license to practice optometry in the Commonwealth of Puerto Rico. Performance period is from date of award for a one year period with two (2) one-year options. The standard industrial code is 8042. A firm, fixed price contract will be issued to the successful offeror. Bid documents available o/a 5/15/98.
