michael.wade@langley.af.mil. HQ ACC/SGOP is hereby soliciting
information to identify sources who possess the capability/experience
to provide Administrative Support Services for Preventive Health
Assessment (PHA). Preventive Health Assessment (PHA) is an annual
health assessment of all active duty Air Force personnel assigned to a
given installation (Air Force Base). The assessment consists of a
series of medical evaluations and reviews which must be collated into
an overall assessment. The PHA administrator utilizes a Microsoft
Access based program to manage this process. The PHA administrator
coordinates with base squadron health care monitors, section
supervisors, individuals being assessed, and the Military Treatment
Facility (MTF) to schedule and track personnel for the PHA process.
This process requires identification of the individuals, multiple
appointment scheduling, and determination of health care provider
(technician and/or physician) appointment requirements. THe PHA
administrator will also accomplish medical record maintenance and flow
management, schedule auxiliary studies (lab, x-ray), notify patients,
and coordinate PHA process interactions within the military treatment
facility (MTF) until completion of the PHA. The PHA administrator will
interact with the base Military Personnel Flight, the MFT Composite
Healthcare Computer System, Air Force PHA systems manager, and other
Air Force agencies to ensure accurate data flow of PHA information. The
administrator will be responsible for generating periodic completion
status reports for the base and higher headquarters. A formal
solicitation does not exist at this time. Responses should include the
following: name and address of firm, size of business and other
literature which demonstrates the capabilities listed above. This

construed as a commitment by the Government nor will the Government pay
