Federer, Contracting Officer, (404) 842-6789 The Centers for Disease Control and Prevention (CDC) contemplates award of a five year contract (including options) to provide overall multi-center coordination and technical assistance to the Diabetes Translational Research Centers in the establishment, analysis, and reporting of a collaborative program of applied population-based research related to diabetes. This collaborative program will develop a knowledge base concerning the assessment of effectiveness, cost-effectiveness, generalizability, implementability, and sustainability of interventions. The multi-center, collaborative, Diabetes Translational Research Initiative within managed-care settings will (1) evaluate the extent to which health-care providers and health-care delivery systems implement accepted standards of diabetes care (e.g., American Diabetes Association standards), which can reduce the burden of diabetes and its complications, (2) explore the factors that affect variations in implementation of diabetes care standards, and (3) develop and test strategies aimed at closing the gap between existing practice and optimal standards of care. The knowledge base to be developed will address methods for and assessment of effectiveness, cost- effectiveness, generalizability, feasibility, and sustainability of interventions. Such a knowledge base may include a variety of activities covering the range of diabetes interventions, including diabetes screening and diagnosis; treatment approaches, glycemic control, CVD risk reduction, and screening for diabetes complications. Written requests for copies of the solicitation will be honored if received within 15 days of the RFP issue date, which will be on or about June 15, 1998. Requests received after that date will be filled on a first-come-first-served basis until the supply is exhausted. The date specified for receipt of offers will be approximately 35 days after the RFP issue date. Requests for copies of the RFP must cite the RFP Number: 98B135(N). Telephone requests will not be accepted. This acquisition will be 100 percent set-aside for small business concerns. The applicable SIC Code is 8733. For purposes of this acquisition, a business concern is considered "small" if their average annual receipts (including affiliates) during the three preceding fiscal years do not exceed $5 Million.
