Z -- CLINIC RESTROOM ADDITION SOL 757-23-98 POC Don Anderson, 614.257.5524 and Nancy Schmidt, 614.257.5525 The successful contractor shall provide all necessary supplies, materials, labor, equipment and site supervision and management to construct a restroom in the Life Support, Observation and Treatment Area in the existing facility. The construction consists of and is not limited to, excavation and backfilling, sanitary drainage, plumbing fixtures, drywall removal and installation, painting, floor and ceiling tile removal and installation, etc. Be advised that the work to be accomplished is in a healthcare facility and all safety measures, the facility's integrity and construction/dust barriers must be fully maintained. All work will be in accordance with drawings and specifications. The estimated value of this procurement is $25,000 to $100,000. Construction must be completed within 30 days after receipt of Notice to Proceed. This solicitation is a small business set-aside, the applicable SIC is 1542, the size standard is $17.0 million annual receipts. All requests for the bid packages must be in writing, indicate the company's size and socio-economic status on the request. There is a $15.00 non-refundable charge for the solicitation package, payment must be in the form of a cashiers check, certified check or money order payable to the VA Agent Cashier. Bid Package will be ready on/about June 12, 1998. There will be a limited number of solicitation packages available, it will be on a first come, first served basis with the proper deposit check.
