(FISC), Norfolk Detachment Washington intends to award a Sole Source
contract to Philips Medical Systems, 7600-G Boston Boulevard,
Springfield, Virginia, 22153. A list of Contract Line Item Numbers and
their items are: CLIN 0001: Maintenance service for two Super CP-100
Generators, Manufacturer: Philips Medical Systems, Model Number: Poly
AU with Super CP-100, Serial Number: 04055; CLIN 0002: Maintenance
service for a Mark V Injector, Manufacturer: MEDRAD, Model Number: MARK
V, Serial Number: 22623. Preventive maintenance shall be performed four
times per year on the CP-100, and two times per year on the Mark V
Injector. Service shall be provided during the months of November,
February, May and August prior to the fifteenth of each selected month.
The contractor shall, at a minimum, perform all preventative
maintenance services as prescribed by the original equipment
manufacturer using a detailed service checklist. The completed
checklist will be provided to the Government with the Contractor's
service report. All test equipment used in the performance of this
contract will be routinely calibrated and the calibration shall be in
compliance with the Joint Commission on Accreditation of Healthcare
Organizations (JCAHO), Original Equipment Manufacturer (OEM) and
Federal Drug Administration (FDA) standards. Maintenance shall be
provided within normal working hours (i.e. Monday through Friday,
between 8:00a.m. to 4:30 p.m.) The contractor shall telephonically
respond within two business hours after receipt of notification of
equipment failure, and shall provide on-site service within eight
business hours. Equipment shall then be operational within sixteen
business hours. Each level of service shall include all parts to be
furnished by the Contractor at no cost to the Government. This
resulting Firm Fixed Price contract will provide (1) one base year and
(3) three one-year options. The Government reserves the right to
extend the term of the contract under FAR 52.217-9 "Option to Extend
the Term of the Contract". All responsible sources may submit cost and
technical information by 26 June 1998 for consideration. SEE NOTE
