Lancaster Rd., Dallas, TX 75216 has a requirement for Community Based Outpatient Clinic(s) in Cooke and Denton counties of Texas. Offerors may proposae among a variety of unrestricted primary care models, for example, offerors may propose a single clinic, network of providers, or community medical center(s). Contractor shall provide for the delivery and management of continuous primary and preventive medical cae for enrolled VA patients. The contractor shall provide health care providers, medical facilities, medical equipment and supplies, pharmaceuticals, and all administration functions in accordance with the terms and conditions of the contract. Compensation will be made through a monthly capitation (per patient, per month) payment. The term of the contract will be one (1) year with two (2) additional one- year option periods.
