Contract base period is one calendar year from the date of award with two (2) one year options for renewal, pending availability of funding. Provider is required to provide global care including all supplies, equipment, employees and resources necessary to provide oncology services. Physicians who will provide radiation therapy consultation must be board certified in this specialty by an American Board of Medical Specialty and apply for clinical privileges at the VA. The provider is required to be JCAHO accredited. Provider's facility must be located within a reasonable geographic perimeter to the VA Medical Center, in order to provide timely treatment for VA patients. Provider is required to access the VA's computer program VISTA in order to enter data into the patient's electronic medical record. This data shall include but not be limited to consults, treatment plans, and assessments. SIC: 8069. All responsible sources may submit a quotation which, if timely received, shall be considered by the issuing agency.
