Offerors may propose a variety of unrestricted primary care models. For example, offerors may propose a single clinic, network of providers, or community medical center(s). The contractor shall provide for the delivery and management of continuous primary and preventive medical care for enrolled VA patients. The contractor shall provide medical facilities, health care providers, medical equipment and supplies, pharmaceuticals, and all administrative functions in accordance with the terms and conditions of the resultant contract. Compensation to the contractor will be made through a monthly capitation payment. Issue date is o/a 6/26/98.
