This Request for Quote is for a Firm Fixed Price Non-Personal Services Requirements Contract. The Federal Prison Camp in Alderson, West Virginia is seeking a qualified individual or organization to provide the services of a Licensed Phlebotomist. Scheduling will normally consist of five sessions per week, with each session being six hours in duration. Hours of work will generally be from 7:00 a.m. to 3:00 p.m. Monday through Friday. The contractor will: draw blood samples, prepare samples for shipment and processing, maintain logs and records, file back up copies of reports, and maintain quality assurance standards as dictated by the Quality Assurance Committee. The services also include performing minor laboratory procedures such as urine dipstick, glucometer readings, hematocrit and fecal occult blood smears. This procurement is a total small business set aside. The contract period is from date of award through September 30, 1999. The estimated availability of this solicitation is July 1, 1998.
