synopsis/solicitation for commercial items prepared in accordance with
FAR 12.6, supplemented with additional information in this notice.
This announcement constitutes the only solicitation; proposals being
requested; a written solicitation will not be issued. A) Solicitation
F41612-98-R0097 is issued as a request for proposal. B) Solicitation
document, provisions and clauses are those in effect through Federal
Acquisition Circular 97-01. C) This is 100% set-aside, SIC 7359, size
standard $5 million, DO Rating S10, FOB destination. Contractor shall
provide all personnel, equipment, tools, supplies, materials and items
to perform work. Work shall be performed by skilled personnel in
compliance with industry standards. New appliances required. Appliances
shall be Commercial Speed Queen Models or equivalent. Washer Model AWM
392W2; Electric Dryer Model AEM497W2; and, Gas Dryer Model AGM499W2.
All appliances in one laundry room shall be of like colors, with
factory baked enamel finish. Washers/dryers shall be a commercial type
from a major manufacturer with a nonadjustable set timer. Capacity
12-14 pound; minimum of delicate and normal cycles; one-speed spin;
porcelain enameled tub; hot, warm and cold water temperatures;
water-level control; user-operated and serviceman-regulated permanent
press cycle; regular cycle; safety lid switch; 120-volt motor and
horsepower rating standard with the manufacturer; and timed-operated
accessible to the user. Dryers shall be gas-fired or electric, and
conform to the following specifications and sizes. Dryer shall have a
12-14 pound capacity with permanent press, regular cycles, an
accessible lint screen and an automatic stop. Gas dryers shall have an
electric ignition for gas burners, gas pressure regulator, safety
thermostat on combustion chamber and a minimum input gas capacity of
20,000 BTU/hr. Flexible gas connectors shall be AGA approved 3/8" to
1/2" to include fittings. Electric dryers shall have nichrome wire
heating coils. Contractor shall submit an installation schedule for all
appliances; deliver/install all appliances in designated buildings at
the existing washer/dryer outlets, including cleaning of all debris
generated from installation within 5 workdays of contract start date.
Contractor shall supply all necessary repair parts, tools, equipment
and vehicles to perform requirements, including all accessories for
proper installation. Contractor shall furnish/install signs,
decals/instructions at each machine. All machines will have a unique ID
#/telephone # for service call, (not the contractor phone #) EMCS --
676-4502. The contractor shall provide name of contractor's
representative/alternate with authority to cope with all
operations/emergencies. Response to all service calls/emergencies is
required within 30 minutes Mon-Fri, 7:30 -- 4:30 (normal duty hours).
Communication equipment shall be utilized to permit contact during
normal duty hours. A complete listing of appliances is required within
10 calendar days after installation. Any revisionis required not later
than 10 calendar days following changes. Listing will contain: Building
number, unique machine ID number, manufacturer's name, model number,
serial number, year of manufacture, and date of installation. A routine
preventive maintenance program is required on each appliance to include
a schedule maintenance on a weekly basis (day of week and times of
schedule maintenance) to include service to all lint collectors weekly.
Contractor shall clean exterior/interior of each machine, clean lint
from all collectors on a weekly basis and all trash weekly from behind,
between, and under machines. An adequate number of serviceable
appliances is required at all times. Scheduled maintenance shall be
sufficient to ensure appliances are maintained in a new or like new
condition, parts free of corrosion, belts must be serviceable.
Maintenance schedule shall comply with manufacturer's recommendation.
Dryer duct work shall be cleaned quarterly to include removal of
wet/dry lint. Wall vents shall be kept free of lint. A duct work
cleaning/maintenance schedule is required not later than 10 days after
contract start date or any change. Schedule shall include date/times
for each laundry area. Contractor shall check with the CE EMCS Section,
Building 1402, at start of each normal work day to receive service
calls. Contractor shall repair or replace damaged or unserviceable
machines with a fully operational machine prior to end of each duty
day. A stock of sufficient supplies of orutine parts shall be
maintained. The contractor shall test all repaired appliances for
operation. All machines requiring four or more repairs within 30 day
period may be removed/replaced as directed by the contracting officer,
replacement shall be made within eight hours of notification.
Contractor shall indicate all corrective action, and sign/submit
service request to Government's designated representative. Service
Tickets: Consecutively numbered service tickets is required on a daily
basis to include contractors representative, ID of laundry area,
appliance number, date/time call was received at EMCS, date/time
contractor received service call, date/time contractor's representative
arrived and departed laundry area, name/phone number of individual
reporting appliance, description of problem, and thorough description
of corrective action taken, and date/time of completed action. All
completed service call tickets shall be submitted to the Government
representative as repair work is completed. Contractor shall relocate
installed appliances as directed by the contracting officer and report
any damage shall be repaired or replaced, as directed in the bid
schedule. Contractor representative shall immediately notify the
Government designated representative of any problems with laundry areas
that are the Government's responsibility; i.e., clogged drains, no gas,
faulty faucets, etc. Compliance with Environmental Laws: Contractor and
subcontractors shall comply with all applicable federal, state, and
local laws, regulations, ordinances, policies and standards related to
environmental matters. Contractor and subcontractors shall also comply
with all applicable specific instructions, policies, or references
contained herein. Policies and procedures can be reviewed at the Base
Environmental Coordinators office, Bldg 1402, phone 676-6840. Hazardous
Materials: (HM) are defined in 49CFR 172.101, revised 1 Oct 97, and
40CFR 302-304, revised 1 Jul 97. Absolutely no HM shall be brought onto
Sheppard AFB until that materials is authorized for use by the
Hazardous Materials Management Office (HAZMO), Bldg 21, phone
940/676-1123. Contractor shall submit AF Form 3952, Material Safety
Data Sheets (MSDSs) and other supporting documentation for each HM used
in accordance with SAFBI 32-7001, dated 9 May 94, prior to use.
Contractor shall notify the Contracting Officer and HAZMO upon
encountering any material, not identified in the contract documents,
thought to be hazardous. The government shall be responsible for
characterization, storage, transportation and disposal of waste, if
necessary. Requirement: 412 washers, 44 electric dryers, 355 gas dryers
for 56 laundry rooms. THE BID SCHEDULE SUPPLIES OR SERVICE AND
PRICE/COSTS SERVICES NONPERSONAL: FURNISH ALL LABOR, MATERIALS,
EQUIPMENT AND TRANSPORTATION NECESSARY TO PERFORM RENTAL AND
MAINTENANCE OF WASHERS AND DRYERS AT SHEPPARD AFB TX IN ACCORDANCE WITH
STATEMENT OF NEED. PERFORMANCE PERIOD: BASIC YEAR -- 1 OCTOBER 1998

requirements are generated, they will be added via modification as sub
CLINs to CLIN X005. Site visit to be conducted 24 Jun 98, 9:00 A.M.
local time. Contractors will notify Contract Administrator at
940/676-6888 not later than 23 Jun 98. Site visit to depart from Bldg
1664. All FAR provisions are available in full text upon request. FAR
Provisions FAR 52.212-1, Instruction to Offerors-Commercial Items; and
FAR 52.212-2, Evaluation-Commercial Items, apply to this acquisiton.
(a) The Government will award a contract resulting from this
solicitation to the responsible offeror conforming to the solicititon
which will be most advantageous to the Government, price and other
factors considered. Award will be made to the offeror who represents
the best value to the Government. The basis of award will be in the
areas of technical, price and past performance. Past performance and
technical are equal in value and are of more importance than price.
TECHNICAL PROPOSAL AREA: CRITERIA 1. Equipment and Supplies: The
proposal shall contain suitable equipment intended for use during the
life of the contract to meet al requirements to accomplish the work,
list all information pertaining to all proposed equipment, including
but not limited to: quantity, nomenclature, capability, age, brand, and
replacement or upgrade schedules. CRITERIA 2. Contractor Representative
and Contractor's Schedules: Proposal shall indicate the method of the
contractor representative's available to Government personnel to ensure
contractor response within 30 minutes. Proposal shall contain a viable
plan for scheduled maintenance to ensure all contract requirements are
met. PAST PERFORMANCE AREA: Offerors shall include all company
affiliation names associated with the company. Past performance must
include: Contract number,whether a government or a commercial contract,
dollar amount, point of contact, telephone number, and type of work
performed for all contracts in the preceding four years. Offeror
include a completed copy of the provisions at FAR 52.212-3, Offeror
Representations and Certifications-Commercial Items, with its offer.
These are required for award of a Government contract. Provisions at
FAR 52.212-4, Contract Terms and Conditions-Commercial items, apply to
this acquisition. The provisions at FAR 52.212-5, Contract Terms and
