Q -- ORAL SURGERY CLINICS AT THE COLVILLE INDIAN HEALTH CENTER SOL RFQ-8044 DUE 072498 POC Contact Ms. Mika Asher, Procurement Tech., 503-326-2012 Contractor to provide Oral Surgery Clinics to eligible Indian Health Service patients at the Colville Indian Health Center located in Nespelem, Washington. The Contractor must have a Doctor of Dental Medicine (DMD), Doctor of Surgery (DDS) from a school within the United States or Canada that is approved by a recognized accrediting body in the United States in the year of the applicant's graduation. Contractor shall maintain a current and unrestricted license to practice medicine in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States of America. Contractor shall have a minimum of three (3) years graduate training in Oral Surgery and possess a Certificate in Oral Surgery. Contractor will be required to provide two (2) clinics per month for eight (8) hours each clinic, over a one (1) year period, for a total of twenty-four (24), eight (8) hour clinics. This solicitation is set aside for small business concerns as defined by Federal Acquisition Regulations. Anticipated contract will be a firm-fixed price contract for non-personal services.
