X -- REAL ESTATE LEASE SOL RFQ 618-135-98 DUE 070198 POC Janice Byrd 612-725-2188 The Minneapolis VAMC intends to award contract(s) for a Community Based Outpatient Clinic (CBOC) and Veterans Outreach Counseling Center in the City of St. Paul, MN to provide Primary Care Services on an outpatient basis for approximately 60-70 patient visits per day and for approximately 16,000 outreach patients per year. The CBOC will be responsible for managing the veterans' primary medical care. Referrals for sub-specialty care and hospitalization will be made to the VAMC located in Minneapolis, MN. It is expected the lease will be awarded on or about July 1, 1998 with occupancy on or about October 1, 1998 for a one-year period with two renewal options. Offers are being solicited for approximately 6365 occupiable square feet of office space that is suitable for the delivery of Primary Medical Care and Mental Health Services. Parking to accommodate approximately 60-70 patient visits and 14 employees per day is also required. Responsible sources capable of meeting the Government's requirement may respond by furnishing in writing, information, which substantiates the offerors ability to provide the required services. All responses must be received within 30 days of the publication of this notice. This information will be used to determine if competitive opportunities exist. Since no solicitation document exists, requests for such documents without accompanying information will be considered non-responsive to this request without further consideration.
