The Employee Assistance Program services are to be provided to employees of the Defense Distribution Depot, Norfolk, Virginia (DDNV). CEAP assessment and referral services will be provided within a 15 mile radius of DDNV. All interested parties shall obtain a Request For Quote (RFQ) from this office by forwarding their written request for the RFQ. All correspondence in regards to this procurement action shall bear: (a) Purchasing Agent's name (Nancy Day); and (b) the Requisition Number (SB3100-81591002). The written request for the RFQ must be received by this office not later than close of business July 1, 1998. Requests received after this time may not be considered.
