The Program Support Center, on behalf of the District of Columbia Department of Health (DCDOH), is seeking sources capable of developing and implementing a program for outreach, referral, placement and interpretive services for hearing-impaired substance abusers who are residents of the District of Columbia and the metropolitan area. The outreach program shall: (1) assess the hearing-impaired needs; (2) educate/increase the hearing-impaired knowledge of the availability for health care resources and substance abuse services; (3) establish contacts with a wide range of organizations and agencies (in addition to the Addiction, Prevention and Recovery Administration) such as hospitals, clinics and social welfare organizations for the purpose of referral and/or placement; (3) refer and place clients to the appropriately assessed organizations for treatment; and (4) provide follow-up support services to the clients. Offerors possessing the required expertise and resources to provide the aforementioned services are invited to submit a complete capabilities statement which demonstrates their ability to satisfy this requirement. This is not a request for proposal and no solicitation package is available. The Government will not pay for the information solicited or received as a result of this synopsis.
