Supplies are required for a base and four (4) one (1) year pre-priced option years. Item (1) Toray Dialyzer, Stock Number B3-2.0A, estimated quantity of 825 cases per year; Item (2) Toray Dialyzer, Stock Number BK-2.1U, estimated quantity of 150 cases per year. These items are manufactured by Toray Industries, substitutions will not be accepted. Delivery will be made on an As Needed Basis. Solicitations will be available on or before June 19, 1998.
