Solicitation number Request for Quotation (RFQ) 673-5-99 is assigned and shall be referenced on any request for solicitation. Solicitation shall be issued on or about July 13, 1998. The James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, Fl 33612, has a requirement for complete maintenance agreement to maintain equipment at the Department of Veterans Affairs Outpatient Clinic, 5201 Raymond Street, Orlando, FL 32803. The duration of this contract shall be from October 1, 1998 thru September 30, 1999 and there will be two (2) one (1) year options. Contractor shall provide all labor, materials, parts, tools, travel, and equipment necessary to maintain the following equipment in accordance with manufacturer's recommendations: 2.1 Radiographic and Fluoroscopy unit, Mfg: Philips, PMI 6525-5457, NRXN: 190287, SN: 903347, Room1097. Consisting of the following equipment: Super 80 CP Control and Generator, Diagnost 66/ Scopo 76, CS62, SCP, BWS and all other Radiographic/Fluoroscopy components associated with this system. 2.2 Radiographic and Fluoroscopy unit, Mfg: Philips, SN 933394, PMI 6525-5522, NXRN 23047 Room 1105. Consisting of the following equipment: Super 80CP Control and Generator, Diagnost 76+/Scopo 76, 100mm Camera, CS62, SCP, BWS and all other Radiographic/Fluoroscopy components associated with this system. 2.3 Radiographic and Tomographic unit, Mfg: Philips, SN 91315, PMI 6525-5456, NXRN 19572, Room 1079. Consisting of the following equipment: Super 80CP Control and Generator, Horizontal Diagnost H, CS62,SCP,BWS and all other components associated with this system. 2.4 Philips Mammo Diagnost 3000, PN: 9804068 1000P,SN:380032, VA:673 EE25995, Room 1088. 2.5 Philips Medical CT Scanner, Model SK7000 SN: KB10102702, EE32056 2.6 Medrad Angiographic Injector, Model: Envision, SN: 10996, EE32057. Successful offeror shall be able to obtain any manufacturer specific software and/or parts for total maintenance of each model of equipment described herein. Successful offeror shall have manufacturer trained personnel. The Standard Industrial Classification Code is 3844 and the small business size standard is 500 employees.
